To apply for Anthem TONIK please click on the link below. You must follow these instructions when applying.
Click then Anthem button below
Select Colorado as the state in upper left corner (do not click apply now)
Below products click Tonik Health Plans
Choose the plan you want and continue with the application
To apply for any other Anthem plan (Smart Sense, Blue Preferred, Right Plan 40) click on the Anthem logo below, then click apply now and complete the application.
